In this presentation, we will explore the history of the Patient Navigator service in Quebec City, tracing its evolution from a pilot project through a full-time service for English-speaking Quebecers arriving in the city for specialized medical services. The new Patient Navigation Network, based largely on the methods and achievements of the Quebec City service, will be introduced, as well as how older adults and caregivers can access this service.
