We kindly ask you to complete this 10 – 15 minute survey by January 24, 2025. Please feel free to share the link with your family, friends, and contacts.

Respond by January 17th to be entered in an early bird draw for a chance to win a $50 gift card to a restaurant of your choice. All responses received by the closing of the survey on January 24th will also have a chance to win a $25 gift card to a restaurant of their choice.

Your responses will remain confidential and a report will be produced and shared this spring.
